Newbie: Solo New Practioner Request for Sample Fee Agreement

I have searched the files and find no sample fee agreements. I am a new solo practioner in consumer bankruptcy in need of support. I seek to file chapter 7 and 13 for now.
I have the 527(b) disclosure forms for my clients to sign, but I am in need of a model fee agreement.
Your help is greatly appreciated.
