Chapter 13 mandatory notice
It is the NOTICE OF HEARINGS which include notice of the 341(a) and the Confirmation Hearing. It is a mandatory form.

What is this mandatory notice referred to in item 5? Is this the notice of the plan confirmation hearing referred to in Local Rule 3015-1(b)(3)?
