Filing a Document Under Seal?

i recently made a motion to be relieved as counsel and didn't want to reveal the reasons so i stated in my dec that the documents were attached as exhibits to the court's copy only. it hasn't been heard yet, before lax, so if you want, i'll let you know how it comes out.

Has anyone ever filed a document "under seal"?
It seems to be a motion under Rule 7026(c), but what are the grounds?
Do you file the motion with the declaration attached?
What if the motion is denied? Does the declaration get filed with the
Court or returned to the moving party?
How do you persuade the Court to permit you to file the document without
revealing information which you are trying not to reveal?
Any help would be appreciated.
